Instructions:

1. Select the name of your clinical educator to access the practicum evaluation page.
2. Enter the password provided to you in your evaluation packet.
3. Complete the evaluation (please note that the items are organized with disagree strongly as the first choice).
4. Print your confirmation sheet and submit it to Kathy Mann.
5. Complete one evaluation for each clinical educator who served as your mentor.
